List providers and dates

Create one row for each provider, appointment date, record requested, bill received, insurer statement, and amount you personally paid.

Keep records and bills distinct

A clinical record describes care; a bill describes charges; an explanation of benefits describes insurer processing. Retain each type so they are not mistaken for one another.

Record gaps without explaining them yourself

If care was delayed or interrupted, note the dates and tell the lawyer what happened. Avoid turning your own assumption into a medical conclusion.

Flag public-benefit or repayment issues

Tell the lawyer if Medicare, Medicaid, a health plan, or another payer may have paid injury-related expenses. Repayment rules are technical and may affect settlement accounting.
